Three workers were hurt Friday, with two suffering life-threatening injuries, when they plunged from makeshift scaffolding at a construction project near San Francisco's Twin Peaks that city officials said lacked permits and violated a number of safety codes. The building's owner has a history of flouting permit procedures and employing unsafe practices, according to the city's Department of Building Inspection.

A construction worker is dead after a fire escape he was standing on broke loose and crashed to the ground. Colorado Springs authorities say the worker fell 15 to 20 feet on Wednesday. He died before he could be taken to the hospital. Authorities say the man was working on a roofing project. The victim’s name has not been released.

A worker was killed when he fell from a scaffold at an apartment complex under construction in North Raleigh late Thursday morning. The man fell from a five-story scaffold, according to a spokesman for the state Department of Labor. Investigators have not yet made public the name of the worker or the company that he worked for, the spokesman said. The incident occurred at 11:08 a.m. in the 5700 block of Goodstone Drive, near Spring Forest and Louisburg roads. The N.C. Department of Labor is investigating.

Three contract workers were injured Wednesday in an accident involving a crane inside a fenced-off construction area on the south side of Talley Student Center at N.C. State University, according to a university spokesman. The three men were taken for treatment to WakeMed Raleigh, according to a university spokesman. One reported a head injury, one reported a foot injury and one had back and neck pain, he said.

A 55-year-old construction worker was killed Monday morning at a job in Windsor Township, York County. Police identified the victim as Benedict Todt, of Gardeners, Adams County. Todt was working at the Red Lion Water Authority pumping facility at 500 Gebhart Rd. when the accident happened around 11 a.m.

A massive pile of salt used to clear icy roads has fallen on a worker at a suburban Philadelphia storage facility, crushing and killing him. Falls Township police say the accident happened Thursday afternoon hours before a massive snowstorm was set to arrive in the area. The 100-foot-tall pile of salt toppled over and trapped the man, who was working a backhoe. Rescue crews were dispatched but couldn't save him.

A 32-year-old Trinity man died early Saturday from injuries sustained in an accident that occurred on a construction site. Michael G. Pavlak was transported to a local hospital where he was pronounced dead. According to a Tampa Police Department report the deceased was working with a crew on the Crosstown Expressway when he was struck by a dump truck. The incident occurred at approximately 4:45 a.m. The death is being classified as an industrial accident.
