A motorist from Franklinton was booked with DWI after causing a head-on collision Tuesday night on Louisiana 25 north of Covington, State Police said. Heather Crockett, 37, who was not wearing a seat belt, received moderate injuries and was taken by ambulance to St. Tammany Parish Hospital, police said.

The driver of the second vehicle was identified as Steven Wallace, 59, of Abita Springs. Wallace, who was wearing a seat belt, received minor injuries and refused on-scene medical treatment, police said.

Crockett was driving a van north on Louisiana 25 at about 8:30 p.m. when, for reasons under investigation, she crossed the center line and struck a southbound Chevrolet Camaro driven by Wallace at the M P Planche Road intersection in the Lake Ramsey area, a Troop L spokesman said.

Impairment is suspected on Crockett's part, and a blood sample was collected, police said. The results are pending.

Crockett also was booked with improper lane usage and failure to wear a seat belt.

A Franklinton woman riding an ATV died Wednesday after she disregarded a traffic signal and was struck by a car, State Police said. The crash occurred at the intersection of Louisiana highways 450 and 16, State Police said.

Cynamon A. Warden, 21, died of injuries at Interim LSU Hospital in New Orleans.

State Police said the collision happened around 12:55 p.m. Wednesday after Warden, who was driving the Kawasaki four-wheeler on Louisiana 450, disregarded a flashing stop sign and red signal light as she crossed Louisiana 16.

The St. Tammany Parish Sheriff's Office said it is investigating the death of a 58-year-old triathlete who died Monday after being struck head-on by a vehicle while he was riding his bicycle. The crash occurred around 2:30 p.m. Sunday.

Raymond Unland Jr. was riding on Riverland Drive in Tchefuncta Estates south of Covington when he was struck head-on by a Jeep Wrangler, authorities said. The Jeep was being driven by a 16-year-old boy. Citing the age of the driver, the Sheriff's Office did not release his name.

A motorist from Jefferson died Monday night from injuries he received in a three-pickup-truck crash on Louisiana 25 near Folsom, authorities said. A driver from Mount Hermon was booked with vehicular homicide, DWI and improper lane usage.

The victim was identified by State Police as Gary White, 66. He died at LSU Interim Public Hospital in New Orleans. The other drivers were not injured, police said.

A two-vehicle crash killed one person and injured two others on State Road 25 in Washington Parish.

Authorities with Louisiana State Police said the crash happened just before 6 a.m. on S.R. 25 when a Ford F-150 pickup driven by Charles Toon, 35, of Foxworth, Miss., crossed the center line and hit a Chevrolet Silverado.

The driver of the Silverado, Jonathan Bickham, 32, of Franklinton, was hit head-on and died of his injuries. He was wearing a seat belt.

Investigators said Toon may have been drinking, and a blood sample was taken. Results are pending.

A motorist who died Tuesday after crossing the U.S. 190 median near Mandeville and slamming into oncoming traffic had "significant" heart disease that may have played a role in the accident, St. Tammany Parish Coroner Charles Preston said. Both Gordon Johnson, 54, of Covington, and a second motorist, Corinne Talbert, 43, of Madisonville, died of injuries they received in the accident, Preston said, but Johnson had heart disease that could have been a contributing factor.

Dennis W. Swedberg Jr. of Pearl River was driving a Dodge pickup truck north on Archie Singletary Road near West Ridgewood Drive at about 8:30 p.m. Sunday when the vehicle ran off the road in a curve. After striking a culvert, the pickup vaulted, ejecting Swedberg before landing back on the roadway.
