The driver of a cement truck killed Friday, May 16, when a freight train struck his vehicle at an unguarded railroad crossing at Green River Golf Club has been identified as an Ontario man.

Michael Cole, 55, was pronounced dead at 12:35 p.m., according to the San Bernardino County Coroner’s office.

The BNSF train struck the Robertson’s Ready Mix truck at a crossing about 12:40 p.m. just north of the club in the 5200 block of Green River Road. The truck was delivering cement for a pipeline project, said Orange County spokeswoman Jean Pasco.

Four people are dead after a collision in Hesperia on Sunday, the San Bernardino County Sheriff's Department said.

The crash happened at Main Street and Balsam Avenue shortly before 7 a.m. The sheriff's department says a van heading eastbound crossed over into westbound lanes for unknown reasons. The van hit a Honda and landed on top of it.

A man and a male juvenile died at the hospital. A woman and a female juvenile were pronounced dead at the scene. Their bodies remain trapped in the Honda.

A woman crossing a street in Riverside was struck by a vehicle and killed on Thursday. Police are investigating. Authorities responded to the accident in the area of Alessandro Boulevard and Mission Grove Parkway at about 7:40 a.m. Witnesses told police that a 60-year-old woman was crossing the street without using a crosswalk when she was struck by a gold 2004 Ford Explorer. The pedestrian was rushed to Riverside Community Hospital, where she was declared dead. Her name has not been released.

Interstate 40 was shut down in both directions in the Ludlow area in the Mojave Desert following a massive gas spill.

A semi-truck carrying liquefied petroleum gas liquid natural gas (according to Accident Data Center comment from Ken below) drifted into the median, landed in a ditch and overturned, spilling the gas.

Caltrans said the freeway is expected to be shut down until at least noon between Hector and Ludlow roads.

The route is heavily traveled by semi-trucks. Area residents reported smelling the spilled gas, but no evacuations were ordered.

An investigation is underway into a deadly hit-and-run crash early Wednesday morning in Fontana. The incident occurred around 1:30 a.m. at San Bernardino Avenue and Commerce Drive. Witnesses say a man was crossing the street, not in a crosswalk, when a semi-truck hit him and took off. The man was pronounced dead at the scene. Investigators have not released the victim's identity - only that he's in his 50s. So far, there are no details on the semi-truck driver.

Drunk driving is believed to be the cause of a deadly wrong-way crash involving three vehicles in the Winchester area of Riverside County Saturday night. The fatal incident occurred at the intersection of Leon Road and Domenigoni Parkway around 10:10 p.m. Gerardo Cornejo, 20, of Nuevo was driving a 1994 Honda Civic westbound in the eastbound lane when he slammed head-on into a 2007 Suzuki RX4. That's when the 20-year-old spun around, and was hit by a Dodge Ram pickup truck.

Authorities are looking for a semi-truck driver they believe is behind a deadly hit-and-run crash in the Fontana area.

California Highway Patrol officials say the semi was heading northbound on Etiwanda Avenue at approximately 2:15 a.m. when it ran a red light at San Bernardino Avenue.

Authorities say the semi struck an SUV and a small Ford sedan. The two aboard the SUV suffered minor injuries, but the driver of the Ford died on impact.

A tow truck driver was critically injured after a hit-and-run driver slammed into him on the side of the road in Bloomington. The incident occurred shortly before 2 a.m. Tuesday when a tow truck driver with Aslan Towing out of Riverside was called to pick up a yellow Chevy Camaro on a bridge over rail road tracks near the 10200 block of Cedar Avenue. The road is very narrow and has no shoulder. As the tow truck driver was putting the Camaro onto the flat bed of his truck, California Highway Patrol officials say someone driving a silver Nissan Altima slammed into him at high speed.

A 15-year-old girl from Murrieta was in a coma and on life support following a skateboarding accident Sunday. The community is now rallying to help raise funds to pay for her medical expenses. Madison Olson was skating down a steep street in San Clemente, when she lost control. She was not wearing a helmet and suffered a serious head injury. Her father, Jason Olson, has faith she will survive, but realizes there will be huge medical expenses.
