Phoenix, AZA fourth person has died after a collision caused by a suicidal driver who fatally shot himself while he was driving. 29-year-old Swapna Ponnambalam died Tuesday at Flagstaff Medical Center. She was in a car with her husband and her mother driving on state Route 89A Sunday when a car driven by 48-year-old Ross "Scott" Willoughby crossed the center line and struck the family's Toyota Prius head-on.Willoughby died when he shot himself before the collision occurred. Authorities say 33-year-old Moorthy Somashundarum and 66-year-old Bhuvaneswari Ponnanbalam were deceased at the crash site.

Phoenix, AZThe cyclist, a 39-year-old from Santa Cruz, was riding east on the bridge on the westbound side of Soquel Avenue, against the flow of traffic, and the pickup was traveling east when the collision occurred. Witnesses said the pickup driver accelerated, causing the tires to squeal and drove on the wrong side of the road in order to hit the man on the bicycle. The cyclist was taken to the hospital with life-threatening injuries. Jeremy Anecito was arrested on suspicion of attempted homicide.

Phoenix, AZA man has been killed in a wood chipper accident in Chandler. Firefighters confirmed one person was pronounced dead. The victim's identity has not been released at this time. One other person was with the man, and while the accident is under investigation, authorities currently believe that it was an accident.
