Hegstrom said witnesses told investigators one of the tires on the Explorer separated causing the vehicle to roll.
Woman, kids hurt when tire comes off, car rolls in Wittmann
When the tire came off, the vehicle rolled and the woman was ejected. She was taken from the scene by helicopter with life-threatening injuries.

Though the children were taken to Phoenix Children's Hospital and are all expected to survive, the woman was flown to a hospital and sustained life-threatening injuries after being ejected from the vehicle.
