Katrina N. Chambers, 24, died at the scene after her 1992 Toyota Camry, which was going west on eastbound I-64 at mile marker 134.5, hit a 2007 Chevrolet Avalanche head-on about 12:10 a.m. Thursday, state police Sgt. Steve Vick said.
