Lafayette, LAYvette M. Ford of Lafayette was northbound in a 2004 GMC Yukon on Surrey Street when she crashed head-on with a southbound semi, according to Lafayette Police said. Ford was transported to a local hospital, where she later died. The truck driver is being treated for serious burn injuries.
