A fourth person has died after a collision caused by a suicidal driver who fatally shot himself while he was driving. 29-year-old Swapna Ponnambalam died Tuesday at Flagstaff Medical Center. She was in a car with her husband and her mother driving on state Route 89A Sunday when a car driven by 48-year-old Ross "Scott" Willoughby crossed the center line and struck the family's Toyota Prius head-on.Willoughby died when he shot himself before the collision occurred. Authorities say 33-year-old Moorthy Somashundarum and 66-year-old Bhuvaneswari Ponnanbalam were deceased at the crash site.

Authorities say a family from California and a 3-year child were killed in a head-on collision on Interstate 40 east of Flagstaff. Arizona Department of Transportation officials on Thursday identified 23-year-old Michelle Bernadet Yaeger, 66-year-old Bruce William Yaeger and 68-year-old Elna G. Yaeger among four victims who died at the crash scene. The Canoga Park, California family was traveling about 15 miles east of Flagstaff. ADOT spokesman Raul Garcia says 3-year-old Kyler Lee Jensen was one of four Winslow family members in the other car and died at the scene.

Three members of an Allen, Texas, family were killed when their van collided head-on with a car on a highway in a remote area of northern Arizona. The Arizona Department of Public Safety identified those killed as 60-year-old Rebecca Simpson Stryker, 56-year-old James Lee Stryker and 16-year-old Travis Stryker. A twin brother of the teenager, 16-year-old Austin Stryker, was seriously injured and hospitalized in Flagstaff. The DPS said a car that crossed the center line first collided with a second car before it collided with the Strykers’ van.
