15-year-old Erin Mathenia of Park Hills died after she attempted to make a left turn off a private driveway onto southbound Highway 21, driving her Dodge Neon into the path of a 2008 Sterling dump truck being driven by 36-year-old David Crisp of Farmington. The truck struck the left side of the Neon, and Erin was pronounced dead at the scene despite wearing her seat belt at the time of the crash.
