Cincinnati, OHA woman died in a crash in Adams County on Wednesday evening.
The Ohio State Highway Patrol said Phyllis A. Walls, 68, of West Union, was traveling southbound on Brush Creek Road at about 7:30 p.m. when she struck an oncoming pickup head on.
Walls was pronounced deceased at the scene by the Adams County coroner.
The pickup driver, Mary A. Channell, 69, of Manchester, was airlifted to UC Medical Center for treatment.
The crash remains under investigation.

Jacksonville, FLA 3-year-old died in a crash in Putnam County on Sunday morning.
It happened on U.S. 17 at Clifton Road near Crescent City around 9 a.m.
Troopers said Gerard Buchan, 74, was driving a 1999 Ford Explorer westbound on Clifton Rd. when he stopped at a stop sign. When he started moving again, he didn't yield to a Dodge Caravan, according to the Florida Highway Patrol report. The vehicles crashed in the intersection.
Ashley Thomas, 29, was driving the Caravan and sustained serious injuries.

Philadelphia, PAInvestigators say a driver is charged with alleged DUI following a three-vehicle chain reaction crash early Friday morning.
It happened shortly after 12 a.m. at the unit block of East Sharpnack Street in Mount Airy.
According to police, the suspect tried to blend in with a crowd that had formed around the accident. He suffered minor injuries.
A 75-year-old man was taken to Albert Einstein Medical Center. His condition is unknown at this time.
Another man suffered a broken arm.
The accident remains under investigation.

Wednesday, December 18, 2013A wrong-way crash on State Road 528 has lanes closed and traffic backed up for five miles in Brevard County.
The crash was reported on westbound SR 528 at mile marker 36 just after 10:30 a.m. Thursday.
The Florida Highway Patrol said a vehicle, driven by an elderly woman, was traveling eastbound in the westbound lanes, sideswiped one car and and then turned around to travel westbound to the Narcossee Road exit.
The car the woman crashed into hit another vehicle, which ignited. The driver of that vehicle suffered minor injuries.

Fargo-Valley City, NDTwo cars crashed head-on in northwestern Minnesota, leaving both drivers dead, authorities said.
The collision in Norman County occurred shortly after 2:10 p.m. Thursday on Hwy. 32 north of Gary, according to the State Patrol.
Killed were James E. Bateson, 66, of Gary, and Doris J. Hanson, 76, of Brooks, Minn.
The crash happened when a southbound car driven by Hanson swerved into the northbound lane and struck Bateson’s car, the patrol added.
The two-lane highway’s surface was dry at the time, the patrol said.

Tuesday, December 10, 2013A 2-month-old child and a pair of adults dodged death when a passing driver clipped their car on an icy road near Klamath Falls on Wednesday and sent both vehicles flying into an irrigation canal, police said.
Howard Kingman, of Sisters, was driving a Chevrolet Tahoe north on U.S. 97 around 1:05 p.m. when he tried to pass a Toyota Prius near milepost 264, Oregon State Police said.
The 70-year-old driver lost control on the icy roadway and struck the Prius, which carried Robert Burton, 56, Bree Burton, 20, and her infant son, police said.

Minneapolis-St. Paul, MNAn 80-year-old man from St. Paul died in a rollover crash the night of Thanksgiving in Dakota County.
According to the Minnesota State Patrol, David Jones was eastbound in the right lane of Highway 110 near Highway 149 when he went off the road and rolled. The one-vehicle crash happened around 8:30 p.m.
Jones was ejected from his vehicle and died from his injuries, the state patrol reported.

Seattle-Tacoma, WAThe Washington State Patrol said a 78-year-old woman caused a crash on Interstate 5 near Mount Vernon when she turned around and started heading the wrong way. The crash happened shortly after 6 a.m. near Anderson Road when the Oak Harbor woman, who was heading southbound, turned around and headed northbound. The state patrol said the woman crashed head-on into a 29-year-old driver in the far left lane.
