Oregon State Police identified the victim of Sunday's fatal car wreck in Yamhill County as 92-year-old Richard Sullivan.

Sullivan, of Portland, was traveling in a car driven by Cathie Sullivan, 67, also of Portland. Police say Sullivan was negotiating a left curve, when her car left the road, traveled off the shoulder and into a ditch where it went airborne and hit the pole.

The driver and her passenger were both trapped in the car and had to be extricated, troopers said.

Richard Sullivan was pronounced dead at the scene.

Police asked for witnesses Thursday to help in their investigation of a two-vehicle crash on Oregon 18 on Wednesday that left three people injured, including a child. A 2005 Chevrolet Astro van driven by Joshua D. Lesmeister, 28 of Tenino, Wash., was headed east near milepost 15 when the van crossed into the westbound lane and crashed nearly head-on with a 2008 Honda Civic driven by Megan K. Goss, 34 of Portland. Lesmeister, Goss and Goss' 18-month old daughter were injured.
