A Tennessee man died in a single-vehicle crash on Interstate 40 Tuesday night. Jason Mitchell, 40, of Milan, Tennessee, died when his vehicle slammed into a ditch. Mitchell's vehicle went across two lanes of westbound traffic before entering the ditch. Mitchell was pronounced deceased at a local hospital.  No other injuries were reported in the crash. An investigation is ongoing.

A Good Samaritan who stopped to help at an accident scene on Friday died after stepping over a bridge railing and falling into a river below. Mark A. Ferguson, 27, of Omaha, Nebraska, most likely believed there was solid ground on the other side when he stepped over the railing and fell into the Stones River. Ferguson was able to swim to the bank of the river, but he was pronounced dead after being taken by ambulance to Vanderbilt University Medical Center.

Two people died in a car accident on Interstate 40 in West Memphis, Arkansas. Six cars were involved in the crash and at least one of them caught fire. Investigators think the weather and construction on I-40 were factors in the crash.  A 13-year-old girl is among those people injured, and is being treated at Le Bonheur Children's Hospital.

Two people were hurt in a three car accident on Interstate 40 East near Poole Road in Raleigh, NC.  The injured were transported to a local hospital.

Authorities responded to a crash on I-40 about 14 miles east of Weatherford just before 8 p.m. Friday. Officials confirmed two people have died as a result of the crash involving a motorcycle and a semi. Oklahoma Highway Patrol (OHP) as for the public's help in finding the semi with an orange trailer, with the word "Allied" written on the side.

Authorities say a family from California and a 3-year child were killed in a head-on collision on Interstate 40 east of Flagstaff. Arizona Department of Transportation officials on Thursday identified 23-year-old Michelle Bernadet Yaeger, 66-year-old Bruce William Yaeger and 68-year-old Elna G. Yaeger among four victims who died at the crash scene. The Canoga Park, California family was traveling about 15 miles east of Flagstaff. ADOT spokesman Raul Garcia says 3-year-old Kyler Lee Jensen was one of four Winslow family members in the other car and died at the scene.

A driver was killed in a crash involving a tractor-trailer on eastbound Interstate 40 on Sunday night.

The two-vehicle collision happened near mile marker 252 in Smith County around 9 p.m.

According to the Tennessee Highway Patrol report, 34-year-old Johnny Burk of Carthage, ran into the back of a semi that was driving in front of him.

The driver of the semi was not injured.

No citations or charges have been filed at this time.

The scene was cleared by 2 a.m. Monday.
