A 16-year-old boy was driving west on I-40 in Flagstaff when he for some reason drove off of the roadway, where the vehicle rolled multiple times. The teenaged driver was thrown from the vehicle and pronounced dead, while a 56-year-old woman who was a passenger was trapped in the vehicle and was transported to the hospital with serious injuries after being extricated. Investigation continues to determine the cause of this accident.

At about 7:43pm, Brewer failed to stop and rear-ended a tractor-trailer that was traveling slower. Brewers car continued off the road, re-entered, and then collided with a pickup truck. Brewer was taken to Washington Regional Medical Center before being pronounced dead, but no one else invloved in the crash was injured.

The construction workers were wearing reflective safety vests and were inside a work zone surrounded by construction barrels, and the construction site was illuminated with portable lighting at the time of the accident. "This is an incident where, late at night like this, more of unfortunate inattention than something we can enforce out here to protect our guys out here," according to First Sergeant Brian Joines.

An El Reno man died Sunday morning after his sports utility vehicle crashed in Sequoyah County, the Oklahoma Highway Patrol reported. About 8 a.m., Timothy Davis, 48, was traveling east on Interstate 40 about a mile from the Roland exit. The front tire on the passenger side on the SUV he was driving had a blowout, and the vehicle went off the road to the right and struck two trees, troopers said. Davis, who was not wearing a seat belt, died at the scene, troopers said.

A person was killed in a crash on Interstate 40 in Seminole County early Wednesday, the Oklahoma Highway Patrol reported. The name of the driver had not been released Wednesday evening. The crash happened about 2 a.m. in the eastbound lanes of I-40 at mile marker 198 about 9 miles north of Seminole, the patrol reported. The driver was ejected and the car caught fire. It is not known if the driver wore a seat belt.
