The investigation determined that Charles Blake Smith, 22 of Mayfield, was operating a 2006 Harley Davidson northbound on KY 534. Smith lost control of the motorcycle in a curve which caused the motorcycle to slide onto its side, cross the center lane, exit the roadway and collide with a tree. Smith was wearing a helmet at the time of the collision.
