Phoenix, AZA 16-year-old boy was driving west on I-40 in Flagstaff when he for some reason drove off of the roadway, where the vehicle rolled multiple times. The teenaged driver was thrown from the vehicle and pronounced dead, while a 56-year-old woman who was a passenger was trapped in the vehicle and was transported to the hospital with serious injuries after being extricated. Investigation continues to determine the cause of this accident.

Knoxville, TNThe teen driver of the van was taken to U.T. Medical Center with non-life threatening injuries. Gaston, who retired in October 2012 after serving 26 years with the Knoxville police department, wasn't wearing a seatbelt.

Oklahoma City, OKA person was killed in a crash on Interstate 40 in Seminole County early Wednesday, the Oklahoma Highway Patrol reported. The name of the driver had not been released Wednesday evening. The crash happened about 2 a.m. in the eastbound lanes of I-40 at mile marker 198 about 9 miles north of Seminole, the patrol reported. The driver was ejected and the car caught fire. It is not known if the driver wore a seat belt.
