A 28-year-old man has been charged after a fatal head-on car crash in Phoenix Sunday evening. Authorities say Octavio Grado was impaired and speeding when he caused a crash on 67th Avenue at 9:15 p.m. The crash occurred when Grado's Nissan Titan hit a curb and crossed the center line and crashed into a Chevy Impala with four occupants. Crystal McDonald, 28, and Isaiah Blanco, 27, died. A 4-year-old girl suffered serious spinal injuries. A 26-year-old man was seriously injured. Initial tests showed Grado's blood alcohol content to be 0.241, according to court documents.

The two-vehicle collision occurred on Milford Street at 3:40 p.m. when a Toyota Sequoia heading southbound crossed the center line and struck a Ford Escape. Toyota driver Thomas Burns, 41, of Milford, was charged with Operating Under the Influence of Liquor. The 31-year-old Ford driver, a female passenger, and two children were transported to a local hospital. Their conditions are not being reported at this time.
