NTSB: Pilot error caused deadly March crash

The pilot's "inadvertent placement of both fuel selectors to the off position" was the probable cause of a Fort Myers plane crash that killed a woman in early March. The National Transportation Safety Board said Tuesday that the placement of the fuel selectors resulted in fuel starvation and a total loss of engine power.

Donna Piehl and Gregory Piehl were pulled from the crumpled plane he was piloting that crashed on its approach to Page Field general aviation airport. Both were listed in serious condition Tuesday, according to a Lee Memorial Health System spokeswoman.

A small engine piper-plane driven by Gregory Piehl went down in a Fort Myers parking lot, about a half mile short of the Page Field runway. Gregroy and his mother Donna, a passenger at the plane, both suffered serious injuries and were hospitalized. Investigators are working to determine the cause of the plane crash.
