34 people injured in subway train derailment between 135th Street and 125th Street in Manhattan, New York

The damage was so severe — with about 200 feet of track and multiple signals damaged and mounds of concrete shorn from the walls — that a track worker said it was highly unlikely that regular service along the line could be restored by the end of the day.

Transit officials said previously that it appears the derailment was caused by an improperly secured piece of replacement rail that was stored on the tracks.
