Wisconsin State Patrol: Driver dies after being struck by piece of truck debris
A preliminary investigation shows a piece of a semi’s brake system fell off the truck — and traveled through the windshield of an eastbound vehicle. That piece struck the driver in the head. The driver died from injuries suffered as a result of this incident.
No suspect vehicle has been located at this time.

Jay Tichelaar, 51, was killed outright in the crash that happened at about 5:10 p.m. in the eastbound lanes, the State Patrol said.

This happened around 5:00 p.m. Monday, May 22nd on I-94 eastbound, just east of Johnson Creek. The semi did not stop and the Wisconsin State Patrol is investigating.
