Police are investigating a single-car crash in Brentwood that left a Northwest Washington man dead. The crash happened Wednesday night around 11 p.m. at the intersection of 38th Street and Rhode Island Avenue. James Kevin Green, 54, was pronounced dead at an area hospital. Preliminary investigation reveals Green was driving westbound on 38th Street when he left the roadway and struck a pole. Speed appears to be a factor in the crash.

An off-duty New York City firefighter is charged with DWI after police say he hit two women, one of them pregnant, while driving drunk in the Bronx Wednesday evening. The incident happened at the intersection of White Plains Road and Westchester Avenue just before 6 p.m. Authorities say 39-year-old Sean Farrell was traveling northbound on White Plains Road when he turned right onto Westchester Avenue and struck the two women, ages 42 and 38, in the crosswalk. Both were taken to Jacobi Medical Center in stable condition. One of the women is seven months pregnant.

A fatal crash occurred Wednesday night in Creek County, the Oklahoma Highway Patrol reports. Arlie Clint Cates, 49, of Bristow, died from injuries suffered in a crash about 11:45 p.m. on State Highway 66 just west of W 271 Street, the patrol reports. Cates died in a Sapulpa hospital after the crash. Passenger Davied Van Orsdol, 49, of Bristow, was taken to a Tulsa hospital where he was treated for a back injury. Seat belts were not in use. Cates was driving east on SH 66 when he drove off the roadway. Cates and Orsdol were ejected.

Eight Stevens County students injured in a bus slide-off accident early Wednesday were evaluated for non-life-threatening injuries in Spokane, according to the Mary Walker School District. Twenty-seven students were aboard school bus No. 6 when it slid onto its side south of Springdale, Wash. All passengers were evaluated by medical personnel at the scene and were either loaded onto another bus or released to parents or guardians, the district said. The roadway was described as a “sheet of ice” by law enforcement on scene during the incident, which occurred around 8 a.m.

Police say a man suffering from a gunshot wound was found inside a car that had crashed and flipped over Wednesday night. Crews responded to the 900 block of Madison Street NW around 8 p.m. for a crash. There, they found a gunshot victim in the car's driver's seat. Police are working to determine where the man was shot. He was not conscious or breathing at time of rescue

Investigations are under way after a Washington Township man was crushed in an industrial accident in Shelby Township today. Erik Deighton, 23, was killed at Colonial Plastics, according to township police and a company statement. “The Roberts’ family and employees of the Colonial Group Companies mourn the tragic death of their fellow employee and friend, Erik Deighton,” according to the company statement from Cheryl Paschall. “The company is conducting an investigation as to the cause of the accident. We have no further information at this time.”

Three people, including a 36-month-old child, were transported to a hospital after a head-on collision in north Phoenix on Wednesday, according to Phoenix fire officials. Phoenix police received the call for the head-on collision between two vehicles near Dynamite Boulevard and Pyramid Peak Parkway just before 11:30 a.m. Wednesday, according to a Phoenix Fire Department spokesman.

Police are searching for one of the hit-and-run drivers who struck a 14-year-old girl who darted into traffic in a busy Orange County intersection. The crash happened Monday in Santa Ana. Surveillance video released Tuesday shows the teen trying to dodge cars at 17th and Bristol streets. First, she is struck by a red pick-up truck, and the driver speeds away. She is then struck again by a black car. Though the video shows her flying into the air and appearing to land on her upper shoulders, the girl suffered leg injuries and has been released from the hospital.

Two Long Island teenagers were killed in an overnight car accident in Roslyn Heights. Police say 19-year-old Steven Clancy was driving 19-year-old Javier Gonzalez when the Volkswagen Jetta they were in crashed into a tree on Roslyn Road near Locust Lane. Both men were pronounced dead at the scene. The investigation into the accident continues. Friends of the best friends say they were out celebrating Gonzalez' 19th birthday.

A big rig flipped off a Bucks County highway this morning causing a backup for hours and state police say it was all caused when an intoxicated driver pulled over to the side of the road. A tractor-trailer collided with a stopped minivan along the southbound lanes of Interstate 95 just past the Woodhaven Road (Route 63) Exit in Bensalem Township, Pa. around 4:15 a.m. Wednesday.
