 An accident investigations board is scheduled to meet Tuesday to decide the cause of a fatal cargo plane crash that has become the focus of dispute between UPS and its pilot union over whether work schedules are inducing fatigue and jeopardizing safety.

The pilots' failures to take certain actions at critical points during their approach to Birmingham-Shuttlesworth International Airport ultimately caused the crash of UPS Flight 1354, the National Transportation Safety Board's acting chairman said.
Two pilots died when plane they were flying crashed into a hill while trying to land at the Birmingham-Shuttlesworth International Airport. They were identified as captain Cerea Beal, Jr., and first officer Shanda Fanning. The National Transportation and Safety Board examined several safety issues, including "dispatch of the accident flight, relevant weather information, flight crew communication, execution of non-precision approaches, adherence to standard operating procedures, flight crew monitoring, fatigue, off-duty time management, and terrain awareness and a warning system."
