Dena Madgel Friedman Van Slyke, 67, died, two others injured in a three-vehicle chain-reaction crash on Old Highway 395 in Bonsall, California

Dena Madgel Friedman Van Slyke was headed north as a passenger in a SUV when a sedan traveling in the same direction veered across multiple lanes of traffic around 5:50 p.m., just south of Old Highway 395, the county Medical Examiners Office said.
