Louis Bernabei Jr. was arrested Friday after investigators said he was on methadone, and his doctor told him he should not be driving.
An 8-year-old girl died in a crash that injured three others on state Highway 85 in Cupertino on Monday afternoon, according to the California Highway Patrol.
The crash was on northbound Highway 85 north of De Anza Boulevard around 3:30 p.m., the CHP said.
The driver of a Jeep Wrangler made an unsafe turn to the right that sent the vehicle to the right shoulder and up a raised dirt embankment, according to the CHP.
The Jeep Wrangler then struck a Mazda stopped at the right shoulder, the CHP said.
The collision sent the Mazda across three lanes where it was also struck by the front of a Jeep SUV before it came to rest facing east on the freeway, the CHP reported.
The 8-year-old girl, later identified as Megan Yang, of Cupertino, suffered fatal injuries when hit by the Jeep Wrangler, according to the CHP.
A 6-year-old Cupertino boy also in the Mazda was transported to Santa Clara Valley Medical Center for major injuries, the CHP said.
A 41-year-old Cupertino woman who drove the Mazda was transported to the medical center for minor injuries, according to the CHP.
The Jeep Wrangler overturned on its roof at the right shoulder and ejected the driver, a 56-year-old San Jose man transported to the center for major injuries, the CHP reported.
The Jeep SUV came to rest on the center divide, the CHP said.All lanes of northbound Highway 85 were closed until about 7:15 p.m., according to the CHP.
The CHP is investigating the cause of the collision.
