Florida Highway Patrol Troopers say Donald Ray Woolfolk II, 48, of Panama City, was driving a 2007 Chevy Silverado when he swerved into the path of a Freightliner Box Truck driven by Wayne Anthony Qualls, 55, of Crestview.
