The 58-year-old man was riding a bicycle south on Pulaski at 10:19 p.m. when he was struck by a white van making a left turn onto Maypole, according to Chicago Police. The van drove away headed west on Maypole.

Investigators released a surveillance photo of the van involved in the hit-and-run crash. It's a white cargo van with a company logo on it, featuring a large letter "A", and the company's phone number. The van may have front-end damage. Anyone who recognizes the logo or has information that may lead to the driver should contact the Major Accidents Investigation Unit at 312-745-4521.
