2 killed, 4 hurt in head-on crash on I-494

The crash happened just before 8:30 p.m. when a 1998 Jeep westbound on Hwy. 5 entered the ramp to 494 and was hit head-on by a 2011 Chevrolet Malibu traveling the wrong way (eastbound in the westbound lanes) on 494. The collision happened in the right lane of westbound 494.

Since 2003, Hayes has been cited seven times for various things including careless driving, speeding faster than reasonable and not driving with care.  Initially in the report, State troopers said alcohol was not detected in the Dec. 2 crash. The results of toxicology tests are weeks away.

Charges: Man with history of epileptic seizures kept driving, killed three in crash near MSP

Patrick J. Hayes, 35, of Savage, was charged Tuesday in Hennepin County District Court with five counts of criminal vehicular homicide or operation stemming from the Dec. 2 crash on Interstate 494 near Hwy. 5 in Bloomington.   

Guilty pleas in crash that killed toddler, 2 others

The Savage man admitted in court that there had been a change in his epilepsy medication and that he shouldn't have been driving the night of the crash in December 2016.
