W. Jordan police say 2-year-old boy died in daycare suffocation accident
A review of video camera footage from the facility, at 7195 S. Redwood Road, reportedly showed the child playing with a bean bag before crawling underneath it about noon Thursday. It is estimated that the boy was under the bean bag for 10-15 minutes before staff discovered him, unconscious and not breathing.

Toddler suffocates after daycare worker sits on beanbag chair he was playing under
According to West Jordan Police, Leonardo was underneath a bean bag chair when a daycare employee accidentally sat on him while reading a book to the rest of the children. Police say he suffocated.
"It just baffles me, makes me wonder, she must not have felt him, she must not have heard him," Sanchez said.

State regulators, though, found three rules violations while reviewing the footage of that day that has put the center on a closer watch. The ratio of caregivers to children was inadequate with two workers looking over 17 children — including eight 2-year-olds, found a report by the Utah Department of Health’s child care licensing division. That shortfall meant insufficient supervision of children, creating the second violation. The toddler who died was out of sight of any caregiver for 20 minutes, causing the third violation.
