Carrillo-Maya hit a southbound Chevrolet Blazer driven by 39-year-old Chad R. Holmes, of Pardeeville. Two other vehicles, a northbound Jeep and a southbound Ford pickup truck were not able avoid the crash, and were also involved in the collision.
