Authorities are investigating a vehicle wreck that killed one elderly woman early Saturday afternoon when a vehicle heading westbound on Glendale Avenue crossed the median and crashed head-on with an eastbound vehicle. The driver of the cross-over vehicle was taken to a local hospital with serious injuries.  A Vehicular Crimes Unit Drug Recognition Expert assessed the driver and observed symptoms consistent with drug use. A fourth person has died after a collision caused by a suicidal driver who fatally shot himself while he was driving. 29-year-old Swapna Ponnambalam died Tuesday at Flagstaff Medical Center. She was in a car with her husband and her mother driving on state Route 89A Sunday when a car driven by 48-year-old Ross "Scott" Willoughby crossed the center line and struck the family's Toyota Prius head-on.Willoughby died when he shot himself before the collision occurred. Authorities say 33-year-old Moorthy Somashundarum and 66-year-old Bhuvaneswari Ponnanbalam were deceased at the crash site.

Donte Caraway, his three daughters, and brother were on the way to the park when a driver crossed the centerline and struck their car head-on. Police believe the driver suffered a medical emergency which caused the crash. Donte Caraway and his youngest daughter, Alana Salgado, were killed in the crash. His brother, Marquise Caraway and two daughters were also injured and hospitalized.

A 28-year-old man has been charged after a fatal head-on car crash in Phoenix Sunday evening. Authorities say Octavio Grado was impaired and speeding when he caused a crash on 67th Avenue at 9:15 p.m. The crash occurred when Grado's Nissan Titan hit a curb and crossed the center line and crashed into a Chevy Impala with four occupants. Crystal McDonald, 28, and Isaiah Blanco, 27, died. A 4-year-old girl suffered serious spinal injuries. A 26-year-old man was seriously injured. Initial tests showed Grado's blood alcohol content to be 0.241, according to court documents.
