Investigators said William Frazer, 64, of West Chester, was driving southbound on I-75 when an unknown black BMV conducted an improper lane change in front of his car.
He swerved to avoid a crash, losing control and traveling off the road, investigators said.

The man and his front seat passenger, Cathy Frazier, 60, of West Chester were ejected from the vehicle. Cathy Frazier died at the scene.
