Employee dies after reported equipment malfunction at plant

Catherine White, of Lebanon, died after an equipment malfunction at her station, according to police. White reported the malfunction to her supervisor, who went to go find someone to fix it. Co-workers later found the 51-year-old woman stuck inside the machine.

I am currently writing a paper on this incident for an industrial safety class. I am curious as to what OSHA findings were on this article and unfortunate tragedy. I have yet to be able to find anything. 

Hi Melissa, In my experience, any OSHA report tends to take months to be completed after an incident, so it may be too soon. I'll keep an eye out and will update this accident page when I find more information. Ingrid, Accident Data Center
