The identity of a woman who died in a three-car accident Tuesday in Beale Township has been released by authorities, according to a report from Pennsylvania State Police Lewistown. Police said Carrie Hackenberger, 28, of Mifflin, was traveling south on state Route 35 in a Pontiac G5 when she tried to turn left onto Academia Road. According to the report, Hackenberger turned into the path of Elyse Patrick, 26, of Lewistown, who was driving a Chevrolet Cruze and traveling north on Route 35.

Salem Police identified the two people killed in a car crash Tuesday evening as a McMinnville mother and her 9-year-old daughter. Kelly Moore, 26, and one of her twin daughters, Jasmine, were declared dead at the scene of the crash, which occurred near the intersection of Turner Road Southeast and Airway Drive Southeast. They were among four passengers in a 1994 Ford Escort driven by Hannah Reitz, 28, of McMinnville.

A driver has been ticketed after a utility worker in Kenosha County was seriously injured when he was struck by a vehicle while flagging traffic, the sheriff's office said Wednesday.

Mary Meyers-Nelson, 73, of Silver Lake was cited for inattentive driving for the accident that injured James Harmon, 46, of Carpentersville, Ill., according to a news release from the sheriff's office.

A 26-year-old woman was hospitalized with a head injury after she was struck by a car in downtown Seattle last night. The woman was crossing the street at the intersection of Denny Way and Dexter Avenue around 7:30 p.m. when she was struck by a Nissan Pathfinder. The SUV driver was evaluated by a drug recognition expert and he did not appear to be impaired, a police spokeswoman wrote in a news release. The pedestrian, who has not been identified, was taken to Harborview Medical Center with what police are calling a potentially life-threatening injury.

A mother has been pronounced dead after the vehicle she was inside with her husband and two small children crashed into a Suffolk pond Tuesday night. Just before 8 p.m., a vehicle made by Nissan in the 6700 block of Burbage Landing Circle left the roadway and traveled into a retention pond, according to the Suffolk Police.

A helicopter airlifted a 23-year-old man to St. Joseph Hospital Tuesday afternoon when a Jeep crashed about 200 feet over an embankment near the Mt. Baker Ski Area. The driver, Taylor Armstrong, 23, over-corrected the Jeep at a left curve in the road, about a mile from the nearest lodge, while coming down from the mountain around 3:35 p.m. One of the passengers, Paul Davis,23, from Normandy Park, didn't have a seatbelt on and was thrown from the Jeep about halfway down. He was airlifted to the Bellingham hospital.

Two inmates in a roadside work crew were struck by a semi-truck on Interstate 5 near Wilsonville Tuesday morning, according to Tualatin Valley Fire & Rescue. Witnesses said the truck driver told them he choked on coffee and then passed out before hitting the workers. One of the injured inmates was taken to Oregon Health & Science University hospital. The 40-year-old woman's injuries were serious, but did not appear to be life-threatening, according to TVF&R spokesman Piseth Pich. The other inmate was treated at the scene. Chris Sturges works nearby and helped the victims.

An SUV crashed through a Larry's Giant Subs restaurant Tuesday afternoon after the driver had a medical emergency, according to Jacksonville police. The crash happened at the restaurant at Lem Turner Road and Interstate 295 on the Northside about 2:45 p.m. Police said the driver in a Honda CR-V was with his wife and stopped at a stop sign, then told wife he was having chest pain. His wife told him to pull over, but the man passed out, slumped over the wheel and pressed the gas, police said. They said the SUV then went into the restaurant.

Las Vegas police are looking for a white company truck involved in a Tuesday morning three-way collision with a moped and a pedestrian in a wheelchair. Police responded about 8:45 a.m. to the accident near Boulder Highway and Russell Road. The pedestrian was transported to University Medical Center with noncritical injuries, according to police. Police would not say how the third vehicle, a moped, was involved in the accident. The truck, which had green lettering on the side, fled the scene.

A vehicle that fled from police collided with a dump truck filled with gravel early Tuesday morning in south Phoenix, according to the Phoenix Police Department. The crash occurred on 19th Avenue near Interstate 17, but the incident originally started at 15th Avenue and Roeser Road when a driver failed to yield to officers, said Sgt. Steve Martos, a Phoenix police spokesman. Officers did not pursue the car but later came upon the collision, Martos said. The driver of the vehicle suffered serious but not life-threatening injuries and was processed for a DUI, Martos said.
