Flags are at half-staff Thursday morning after off-duty Cleveland firefighter Theodore J. Wright, Jr.'s pickup truck crashed into a telephone pole and died.

Cleveland Fire spokesman Lt. Larry Gray says the 32-year veteran was married with children. The crash is being investigated by CPD’s Accident Investigation Unit.

EMS arrived at the scene on E.71st and Claasen Avenue around 3:10 a.m. on Thursday.They said Wright was partially ejected and still trapped inside of the vehicle. He was the only person in the vehicle and no one else was injured.

An elderly Lincoln woman died after a two-vehicle crash early Tuesday evening. Authorities say Viola Garvey, 82, was pronounced dead after the accident in the 2300-block of Woodlawn Road in Lincoln.

The crash remains under investigation by the Lincoln Police Department and the Illinois State Police Accident Reconstruction Division.

A west suburban man was killed in a head-on crash Monday morning in Elburn, police said.

An eastbound 2002 Toyota Camry crossed the center line on Keslinger Road about 7:30 a.m. and struck a westbound 2000 Chevrolet van near Harley Road, according to the Kane County Sheriff's office.

Burton Brown, 68, of Elburn, was taken to Delnor Hospital in Geneva, where he was pronounced dead, police said. He was the driver and only person in the Camry.

A Crystal Lake man was killed and another person was critically injured in a traffic crash involving three vehicles Monday evening in the northwest suburban. A four-door Toyota was headed south on Pyott Road from Rakow Road about 5:45 p.m. when it crossed into the northbound lane and collided head-on with a four-door Honda, according to a statement from Crystal Lake police. The force of the collision drove the Honda into a Chevy Tahoe that had been northbound on Pyott, police said. The driver of the Honda died at the scene, authorities said.

A single vehicle crash with severe injuries occurred at approximately 2:40 p.m. Saturday about of mile north of Lockhaven Road on Illinois Route 100, also known as the Great River Road, in Jersey County. A motorist driving a Chevrolet Corvette apparently lost control and rolled over on Illinois Route 100; no other details were known as to what caused the crash at 11 p.m. Saturday. The driver perished from injuries sustained in the crash.

Authorities say a worker at a farm chemical company died after his fertilizer truck was struck by an Amtrak train in the east-central Illinois community of Savoy.

Champaign County Sheriff officials say the 42-year-old man was from Newman, but his name hasn't been released. He died after being airlifted to Carle Foundation Hospital in Urbana.

Authorities stated that the man was driving a three-wheeled fertilizer truck out of a farm field on Thursday night when he drove in front of the northbound Amtrak train.

The driver of a van that collided with an empty CTA bus has been identified as Eric West, Jr. Three other people were hurt in the crash.

One person was killed and several others were hurt when a van slammed into the CTA bus and burst into flames near South Chicago and Commercial Avenues at about 12:45 a.m. Wednesday, according to investigators.

The parents of the fatal victim said West, 25, was the driver of the van involved in the Calumet Heights crash.

A 48-year-old man has been charged in a crash that injured five people at a Roseland bus stop, following a police chase Tuesday night.

James Pugh, of the 1400 block of West 115th Street, has been charged with two felony counts of aggravated fleeing; and five various misdemeanor counts including DUI, driving on a suspended license, marijuana possession, and criminal damage to property.

A bicyclist was killed after a collision with a van in Roselle, northwest of Chicago, on Tuesday morning.

The bicyclist's name was not publicly released as of 10:15 a.m., pending notification of family members, but Sgt. John Lawson said the man was born in 1966.

The incident happened on South Roselle Road just south of the Elgin O'Hare Expressway. Lawson said the driver of the van traveled eastbound on Rosemont Avenue before turning northbound onto South Roselle Road.

Two people were airlifted to the hospital after a multi-vehicle wreck in St. Clair County, Ill. on Tuesday. According to police, two men were in a vehicle heading southbound on Ill. Route 4 near Jefferson when the driver lost control, overcorrected and slammed head-on into a car heading the opposite direction. The driver of the first vehicle and a woman driving the other car were airlifted to the hospital. The other male passenger was taken to a Belleville hospital before being transfered to a hospital in St. Louis. All three victims are in critical condition.
