An Oklahoma City man died Wednesday afternoon in a crash on Interstate 40 in Caddo County, according to the Oklahoma Highway Patrol. About 5:10 p.m., Kevin Kizarr, 43, was driving west on I-40 when he went off the road at an exit about five miles north of Hinton. He drove up the embankment and the sport utility vehicle rolled, according to a patrol report. Kizarr died at the scene, troopers said. He was not wearing a seat belt.

A man was struck by a semi-trailer on Interstate 39 in Milton Wednesday morning, apparently after leaving his vehicle to retrieve a piece of luggage. The crash in Rock County happened at about 10:50 a.m., according to the Wisconsin Department of Transportation. The severity of the victim's injuries are not known. All lanes around the site of the crash are now open, according to the department.

The Florida Highway Patrol has arrested a Zephyr Hills man and three of his family members in connection with a head-on hit-and-run crash that sent a Pasco County Sheriff's Office deputy to the hospital. Deputy Darren Hill, 29, of Wesley Chapel was seriously injured when his cruiser was hit by a F-150 Ford truck in Wesley Chapel area early Wednesday morning, according to the Pasco County Sheriff's Office.

Emergency workers in La Crosse have recovered a body from a car that broke through the ice on a creek. The driver was the only person in the car. A bus driver called dispatchers Wednesday about 1:30 a.m. when he noticed tire tracks leaving the street and heading over a bank toward Swift Creek. The vehicle plunged down a 40-foot embankment but the ice didn't immediately break. The vehicle headed a little south on the ice before breaking through into the water. Rescue divers found the vehicle submerged in 6 to 8 feet of water. An autopsy is scheduled for Thursday in Madison.

A mother has been charged with driving under the influence after her two young children were killed in a car crash, according to the South Carolina Highway Patrol. Troopers said Melissa Patton, 32, is charged with two counts of felony driving under the influence and driving under suspension. According to troopers, Patton was driving a car at 2:15 a.m. Wednesday on Wells Highway when the car traveled off the road, hit a church sign and overturned.

One person was killed and three people were injured early Wednesday morning in a Burien crash. The collision happened at the intersection of 112th and 12th Avenue before 3 a.m when a vehicle crashed into a tree. There were three men and one woman, believed to be in their 20s, in the vehicle. One person died at the scene and the three others were taken to the hospital with serious injuries. Deputies believe speed and fog were factors in the crash and it is unknown if alcohol was involved.

A 22-year-old Lacey man was killed early Wednesday morning crash in Olympia. The collision happened at about 1:15 a.m. in the 2700 block of Pacific Avenue. According to police, a pickup truck was traveling at a high rate of speed when it left the road and hit a tree. The driver, Alex Petermann, was the only person in the vehicle and died at the scene. Authorities said the truck was traveling between 60 and 100 miles per hour.

Two people were killed when their car veered off a Seattle roadway and rolled, police reported. The crash occurred shortly before 3 a.m. today, when a Ford Mustang convertible traveling north on Lakeview Avenue East, lost control and left the road, a Seattle police spokeswoman reported on the department’s website. The Mustang went sideways and hit a fire hydrant, which caused it to roll onto its roof, Witt said. Seattle fire personnel treated the car’s three occupants, pronouncing the driver and a passenger dead at the scene.

The driver of a pick-up called his girlfriend to complain of chest pains just 10 minutes before his vehicle crossed the center line of State Route 7 and struck another vehicle. The driver is dead and six people from the other car were taken to the hospital after the collision. Both lanes of SR 7 are closed at 316th Street East near Eatonville and traffic is being detoured due to the collision. Two adults were taken to MultiCare Tacoma General Hospital and four children are at MultiCare Mary Bridge Children's Hospital. Their conditions are not available at this time.

One person was killed and three others were injured in a one-car crash on Rutherford Boulevard early Wednesday morning. Police say Robert S. Cook, from Burns, TN, lost his life as a passenger in the car that crashed about 3:30 a.m. on Rutherford Boulevard near Prestwick Drive. According to authorities, the driver, Sheila Holmes, lost control of the car on a curve, hit the center median, then hit a utility pole on the other side of the road. Two other passengers in the car were hurt, and police say another passenger, Nicholas Parker, left the scene and hasn't been found.
